本报告分析了偏远农村贫困留守老人现存的“对象特殊化”“群体碎片化”“院舍空心化”“管理行政化”“生活枯燥化”五个问题，以社会工作的专业视角从微观、中观、宏观三个层面进行需求评估。在此基础之上创新提出“社会工作+”的概念，构建了“社会工作+”介入模式的介入理论体系、介入主体体系、介入形式体系、介入方法体系、介入资金体系、介入评估体系六个体系，并根据“社会工作+”介入模式展开行动研究，进而从四个方面对实务的探索进行呈现。
This report analyzed the remote rural poverty left-behind elderly existing object "specialization" "fragmentation" collective "hollowing out" of home town "management" "bored" five questions, to professional point of view of social work from the micro, meso and macro needs assessment in three dimensions. Innovation on this basis the author puts forward the concept of "social work +", build the "+" social work intervention model interventional theory system, the main body system, interventional form system, method system, six system interventional funding system, assessment system, and according to the pattern of "+" social work intervention in action research, and then from four aspects to present practice exploration.
